Job Title: Personal Attendant
Responsible to: Program Manager
Responsibility to supervise other employees: None
Classification: Non-exempt
Minimum Qualifications:
-
High School Diploma / GED
-
18 years of age or older
-
Ability to speak, read and write in English
-
Proven organizational and time management skills
-
Basic arithmetic skills and ability to balance a checkbook and record expenditures
-
Ability to follow oral and written instructions
-
Knowledge of community resources
-
Ability to work flexible hours, including evenings, weekends and some holidays
-
Valid driver’s license, reliable vehicle and current automotive insurance preferred
-
Required to have current First Aid and CPR training
Care responsibilities:
-
Complies with the company’s rules and state and federal laws regarding consumer care. Carries out duties in accordance with each consumer’s Individual Service Plan (ISP). An ISP outlines the areas in which the consumer needs support and/or training from a personal attendant. An ISP covers various topics, including but not limited to; self-advocacy, safety, health improvement and maintenance, recreation, hygiene, meal planning and cooking, shopping, public transportation, menu planning, money management including banking, medical and dental and other community resources

Executes ISP in an accurate, efficient and thorough manner

Maintains up-to-date accurate records for consumers

When necessary, attends consumer planning meetings to assist with advocacy
-
Regularly communicates with consumer’s family, conservator and interdisciplinary team members
-
Schedules and attends (if needed) medical appointments and arranges transportation
-
Coordinates and/or provides transportation for consumers to and from community activities
-
Regularly monitors consumer’s homes to confirm consumers live in a safe environment

Perform duties in the utmost ethical manner by, maintaining consumer confidentiality

Willingness to receive feedback regarding, and to improve upon work performance

Takes initiative without prompting from supervisors
Physical / Mental Requirements

Bending, crouching, stooping, stretching, kneeling, and reaching or similar activities

Lifting objects up to approximately 60 pounds

Occasional lifting or transferring persons

Sustained alertness and concentration required to supervise consumers
-
Ability to hear and communicate with consumers

Ability to negotiate with consumers
